Debulking Surgery for reducing the bulk

Debulking Surgery for reducing the bulk: Procedure

Debulking surgery involves the removal of as much tissue as possible. The term debulking itself means to ‘reduce the bulk’ of the tumor. It involves removal of as much as visible tumoral growth as possible, leaving no tumor measuring more than 20mm in diameter. It increases the chances of the killing of tumor cells by chemotherapy or radiation therapy.

About the procedure:

Debulking surgery is the standard procedure for the treatment of ovarian cancer. The surgery involves an open incision. Anesthesia is given to the patient to avoid discomfort to the patient. This method is often used before the delivery of chemotherapy so that the drugs can reach the target tumor cells and act more effectively causing a better response to the treatment. The average recovery time is around 2 to 3 months for the patients to feel back to normal.

Debulking surgeries have shown to be an effective treatment option for certain cancers. They are mostly used for the treatment of carcinoma of testes and ovary, renal cell carcinoma, sarcoma, renal and other types of tumors related to endocrine. It is also called cytoreductive surgery when performed for curative intent. This type of surgery is very helpful in treating the late stages of certain cancers.

Some risks in debulking include

Debulking

  • Fatigue
  • Nausea
  • Pain
  • Loss of appetite
  • Hair loss
  • Bleeding
  • Infection
  • Side effects of anaesthesia on the nervous system.

Cryosurgery: freezing and destroying cancerous cells

Cryosurgery: freezing and destroying cancerous cells

Cryosurgery is a type of surgery that involves destroying abnormal tissue with the help of a cooled liquid or an instrument called cryoprobe. A cryoprobe is cooled with substances like liquid nitrogen, liquid nitrous oxide, or compressed nitrogen gas. It generally involves freezing and destroying cancerous cells or cells’ potential of turning into tumors.

Cryosurgery

Cryosurgery is often used to treat cancers. It is also used to treat a variety of benign types of skin problems. It is used when other forms of treatments have proven unsuccessful, especially when cancer has returned following other treatments. It is also used on some internal organs like the liver when conventional surgery becomes difficult or risky.

Cryosurgery is used as a primary treatment for Prostate Cancer. The method is best suited with patients having light skin and for most non-hair bearing parts of the body. It is beneficial because it has a short preparation time, low risk of infection, and minimal wound care. It also does not require anesthesia and or other expensive supplies.

Risks in cryosurgery include bleeding, blister formation, headache, hypopigmentation, and hair loss. There are other risks which include scarring, sexual dysfunction, loss of sensation if nerves are effected and ulcers.

Although cryosurgery carries risks, they are considered lower than other types of treatment methods adopted for cancer treatment.

Staging surgery: Why is it needed?

Staging surgery: Why is it needed?

Staging surgery, also called surgical staging is most commonly performed in endometrial cancer to remove the uterus, cervix, pelvic and para-aortic lymph node tissues. It is an accurate method of determining the extent of the disease. Patients suffering from ovarian cancers are mostly advised for surgical staging.

During the procedure, an incision is made in the abdomen to examine the organs within the pelvis and abdomen for signs of cancer. Samples of tissues are taken from various affected areas and are sent to the specialized pathologist. If the pathologist finds evidence of ovarian cancer in the section, then the surgeon attempts to remove as much cancerous tissue as possible. This is an important step in the treatment of ovarian cancer and is called surgical staging.

Staging surgery

This procedure is performed to access the extent of the disease. Decisions for further treatment of cancer, like chemotherapy, are taken after the surgical staging of cancer.

Although staging surgery involves the removal of all cancerous parts in the ovary, fallopian tube, and uterus, it also includes the removal of lymph nodes and other organs, depending upon the various examinations.

Like all types of cancer treatments, the surgical staging also has side effects. The treatment may affect routine functions at work, home, intimate relationships, feelings, and thought.

Palliative Surgery for improving Quality of Life

Palliative Surgery for improving Quality of Life

Palliative surgery is a type of surgery used to relieve the symptoms of the patients who have no hope of complete treatment. It provides symptom relief and helps the patient to increase the longevity of the life of patients. This type of surgery usually involves the usage of least interventive treatment for increasing the life of people suffering from a terminal illness like cancer.

Palliative Surgery

The palliative surgery is performed to evaluate the extent of disease. The procedure also helps to control the locoregional spread of the disease. It is performed in case of spreading of a tumor, or discharge or hemorrhage. Palliative surgery is often performed to reduce the pain being experienced by the patient. This surgery can range in a variety of operations from debulking to less complex operations.

Some cases when a palliative surgery may be performed are

– To remove the neurolytic blockage in head and neck disease.

– Surgical debulking in case of fungating breast cancer.

– Stent insertion for spinal cord compression.

Like all types of surgery, palliative surgery also carries risks. The patient should consider the risks of the surgery with the doctor. Generally, it is performed to increase the life span of a person. But, some ethical questions have been raised against the procedure. The most important one being about the providing of true information to the patients without destroying hope and complex decision making.

Appendicitis: Sign and symptoms

Appendicitis Causes, Sign and symptoms

Appendicitis is a condition when the appendix is inflamed, swollen, or filled with pus.

An appendix is a pouch-like sac tissue on the abdomen’s right side, connected to the first part of the colon (cecum). It is an area for friendly bacteria. The generic name of the appendix is “Vermiform appendix.”

Appendicitis is caused when the infected part blocks off the appendix’s opening due to mucus, stool, or a combination of two, sometimes. If the blockage is severe, then the appendix’s tissue may die and rupture or bust, which needs medical emergencies where surgery is carried out known as appendectomies( removal of the appendix).

 Sign and symptoms of Appendicitis

  • The very first sign is pain across the abdominal area. (Know about abdominal pain)
  • Cough and sneezing
  • Fever
  • Nausea, vomiting, and diarrhea
  • Loss of appetite

Chronic Excessive Flatulence: Causes

Chronic Excessive Flatulence: Causes

Definition: Excess gas in the digestive tract lead to abdominal discomfort and distress which is known as flatulence. People often pass gas without noticing which is healthy gas and is harmless.

Chronic excessive flatulence occurs when the person swallows tiny amounts of air, this will be released either by belching or through the anal passage. When there is a smell in the flatulence it is because of the small number of sulfur gasses. When the food is not digested properly it starts to decompose leads to the release of sulfur.

Chronic excessive flatulence occurs from the intake of certain foods continuously. For example,

  • Vegetables like artichokes, beans, broccoli, leeks, cauliflower, cabbage, garlic, onions, Brussels sprouts, and turnips.
  • Cereals like wheat and oats
  • Dairy products

Causes

Flatulence

  1. Increased frequency of passing gas
  2. Belching
  3. Abdominal bloating
  4. Abdominal discomfort or pain
  5. Foul-smelling gas
  6. Swallowed air
  7. Breakdown of undigested foods
  8. Lactase intolerance
  9. Mal-absorption of certain foods.

Laparoscopic Surgery for detecting the causes of pain

Laparoscopic Surgery: Procedure and Risk Factors

Laparoscopic surgery is a type of minimally invasive surgery which is used for diagnosing problems in the abdomen or the reproductive system of women. Laparoscopic operation is different and better than “open” surgery which is done by making an incision of several inches long in the abdomen.

Procedure:

Laparoscopic Surgery

A laparoscope is used for the operation, which is a long, slender tube inserted into the abdomen by making a small incision. A camera is attached to the tube through which the doctors monitor the internal organs. If some problem needs to be fixed, other instruments are used. These instruments are generally inserted by making small incisions through the abdomen and are used to treat the problem.

Why is it performed?

Laparoscopic surgery is done to identify and treat the causes of chronic pain in the pelvic region, to remove tumors, blockages, and infertility. It is used to diagnose the cause of unexplained bleeding and infections. It is used to remove an ectopic pregnancy, a pregnancy that grows outside the uterus, and can be life-threatening for the pregnant women. It is also used to perform a hysterectomy, to treat cancer, abnormal bleeding, or other disorders. It can also be used to treat incontinence, which is involuntary or accidental urine leakage.

Laparoscopy can be suggested to a person who:

– Has abdominal cancer.

– Feels a lump in the abdomen.

– Has severe chronic pain in the pelvis or the abdomen.

– Wants surgical birth control.

– Is having trouble getting pregnant.

Laparoscopic surgery has many benefits. It is a minimally invasive surgery. The recovery is generally faster than open abdominal operations due to smaller size and number of incisions. The patient can be discharged in 24 hours. And it has fewer risks of infection.

Risks in a laparoscopic operation include:

– Internal bleeding

– Hernia (bulging caused by poor healing of the incision site)

– Mild abdominal pain after the operation.

Laser Surgery: Why is it a bloodless Surgery?

Laser Surgery: Why is it a bloodless Surgery?

Laser surgery is a type of surgery which involves the usage of focused light, called laser light to perform various surgical operations. A laser stands for ‘Light Amplification by Stimulated Emission of Radiation’. A laser is a focused beam of light, tuned to specific wavelengths.

A laser surgery operation can be used in various types of surgeries to:
  • – Remove or shrink tumors.
  • – Cut or destroy abnormal tissues without damaging normal and healthy tissues.
  • – Cauterize (seal) blood vessels to prevent blood loss.
  • – Remove tattoos, warts, moles, scars, wrinkles, and birthmarks.
  • – Remove kidney stones.
  • – Repair the detached retina.
  • – Treat hair loss involving alopecia or old age.
  • – Close off nerve endings to reduce postoperative pain.

– Close lymph vessels to reduce swelling and preventing the growth of cancer.

Laser Surgery

Different types of lasers are used for different types of surgeries. For example, a CO2 laser is used to make small cuts for making shallow cuts for treating superficial cancers like skin cancer. Nd: YAG lasers are to treat throat cancers. They can penetrate deeper into the tissues and can clot the blood quickly. Laser-Induced Interstitial Thermotherapy (LITT) involves the usage of lasers for heating certain areas of the body. The heat results in shrinkage, damaging and destroying of the tumor. Argon lasers pass through the superficial layers only and are used in Photodynamic therapy (PDT) to activate chemicals in cancer cells.

Laser surgery is often called as ‘bloodless surgery’ because it involves very minimal bleeding than conventional surgery. It often takes less time. It has very high accuracy and requires a smaller incision. The heat generated from the laser keeps the site free from germs and requires less recovery time.

The risks involved in laser surgery include hemorrhage, infection, perforation of organ and tissues. The intended effects may not be permanent and repeated sessions of laser operations may be required.

Diagnostic Surgery for detecting suspicious tumor

Diagnostic Surgery for detecting suspicious tumor

Diagnostic surgery is a type of surgical method often used for obtaining a biopsy, or a tissue sample for diagnosing the presence of cancer. This helps in detecting and confirming the presence of cancer. This involves physically removing a part or whole of the suspicious tumor and its examination under a microscope.

After careful examination of the biopsy, the suspicion of cancer can be confirmed. A diagnostic surgery may vary depending on the organ to be biopsied. Some may require major surgery, while some may not even require local anesthesia. This type of surgery is majorly used for detecting the presence and detect the type of cancer.

The following types of procedures are mainly adopted to do diagnostic surgery

Diagnostic Surgery

– Core needle biopsy

– Incisional biopsy

– Excisional biopsy

– Fine needle aspiration biopsy

Diagnostic surgery can be done using minimally invasive surgical techniques or using traditional open surgery. Doctors can also take a biopsy during endoscopic procedures or using needles to withdraw cells. But there are some uncertainties in this procedure. Though a positive diagnosis indicates a definite presence of cancer, negative diagnosis can either mean no cancer or can also mean that the specimen was not representative of the tumor.

There are some risks in diagnostic surgery, depending upon the type of biopsy and the type of biopsy technique chosen. Some biopsies like core biopsies, needle biopsies do not show positive results because of the less number of cells extracted from the site. In some rare cases, the needle misses the tumor and extract healthy cells to produce an incorrect diagnosis.

The Common Pancreatic Disorders and Treatment

The Common Pancreatic Disorders and Treatment

The pancreas is a compound acinotubular gland located behind the stomach. Common Pancreatic Disorders are

  • Inflammatory Disorders- Acute and Chronic Pancreatitis
  • Hereditary Pancreatitis
  • Malignancy- Pancreatic Cancer
Acute pancreatitis

Acute pancreatitis is an inflammatory condition of the pancreas, with a 10% mortality rate. The diagnosis is often critical, which delays treatments in patients.

The most common cause is gallbladder stone/s. A surgical procedure is needed if imaging studies like X-rays are inadequate. Prevention of recurrent episodes is a priority for a physician, where 17-25% of patients are generally affected.

Causes:

  • High blood levels of triglyceride or calcium
  • Medicines
  • Alcohol- heavy consumption

Symptoms:

  • Mild to worse pain after eating
  • Illness associated with fever
  • Vomiting and nausea
  • Tender or swollen abdomen
Chronic Pancreatitis

Pancreatic Disorders

Chronic Pancreatitis gets worse over a period of time. The inflammation may cause permanent damage to the pancreas, leading to less improvement in symptoms. The disease affects males more often, between the ages of 30-40 years.  The frequency is 50 patients in 1 million populations.

Causes:

  • Heavy alcohol use
  • Autoimmune disorders/condition
  • Blockages in pancreatic duct/bile duct
  • Family history- genetic reasons

Symptoms

  • Severe Nausea and Vomiting
  • Rapid weight loss (even when appetite is normal)
  • Diarrhea or passage of fatty stools
  • Change in color of stools- either pale or clay-colored

Diagnosis:

  • X-ray examination
  • USG- transabdominal/endoscopic
  • MRCP (Magnetic Resonance Cholangiopancreatography)
  • CT-Scan
Hereditary pancreatitis

This type of inflammatory condition is related to genetic or inherited abnormalities of the intestine or pancreas. The recurrent attacks, early in life, may lead to chronic pancreatitis. Cystic Fibrosis, if inherited, often causes chronic pancreatitis. This inflammatory condition is often progressive in nature, leading to long-lasting problems.

Symptoms are diarrhea, moderate to chronic pain, malnutrition. The treatment goal is pain control and replacement of pancreatic enzymes.

 Pancreatic Cancer

It is a condition caused due to abnormal growth and division of mutated pancreatic cells. The pancreatic cells start functioning abnormally. The mutated cells can build up and forms a mass known as a tumor.

A tumor that has cancerous cells becomes malignant. It is very difficult to detect pancreatic cancer in the early stages. By the time the symptoms appear, cancer must have reached in its advanced stages.

Achalasia: The Esophageal Motility Disorders

Achalasia: Types | Diagnosis | Symptoms | Causes

Achalasia is a Greek term derived from two words a- which means not and chalasis, which means relaxation. Achalasia means failure to relax, either muscle, sphincters, or normal functions related to contraction and relaxation of muscles.

Types:

It is of four types, depending on the site of the weakness of the sphincter. They are:

  1. Achalasia of the cardia is the most common type where the cardiac sphincter restricts the passage of food from the esophagus to the stomach. In advanced stages, dysphagia is marked sometimes along with dilatation of the esophagus.
  2. In Cricopharyngeal achalasia, there is a failure of the lower pharyngeal muscles to relax while swallowing. This condition in later stages causes dysphagia or difficulty in swallowing and sometimes can also cause aspiration of food.
  3. Pelvirectal achalasia results due to the congenital absence of ganglion cells in the distal part of the large intestine. This further leads to the failure of the colon to relax.
  4. The sphincter of the intestines fails to relax in Sphincteral achalasia.

How to diagnose achalasia?

The following procedure is followed to diagnose achalasia:

  • Endoscopy,
  • Barium meal x-ray, or
  • Oesophageal manometry

Symptoms of achalasia cardia:

Achalasia

  • Dysphagia or difficulty in swallowing food, either solid or liquid
  • Regurgitation of food especially at night
  • Aspiration of food to lungs which causes further complications
  • Pain in chest
  • Heaviness and discomfort around the chest
  • Heartburn is not relieved by medicines.
  • Loss of weight
  • Cough at night
  • Retention of saliva at night
  • Dryness of eyes and mouth

Causes

The specific cause is not known, however some causes that have been known to cause this condition are:

  • Auto-immune
  • A rare parasitic infection is known as Chagas disease.
  • Degeneration of nerves near the cardiac sphincter.
  • Cancer of the esophagus
  • Family history

Risk factors:

  • Middle-aged and older adults
  • Obese people
  • People with a family history of achalasia
  • People living in South America as the parasite, which causes Chagas disease is quite prevalent there.

Complications:

  • Pneumonia
  • Lung infection
  • Cough
  • Cancer of the esophagus.
  • Tearing of the esophagus
  • Acid reflux

Lifestyle aids to manage symptoms:

Dietary changes can help to manage symptoms, although the diet cannot help to treat the disease. The following dietary changes help to relieve symptoms:

  • Drinking plenty of fluid and water
  • Taking a small meal with lots of fluids
